Advanced Practice Clinician - Nurse Practitioner or Physician Assistant

Job Details:
• Part-time position (0.25 FTE, 10 hours per week) at 11th Avenue Medical Center • Utilize EPIC electronic medical records system • Work with certified bilingual Medical Assistants dedicated to assisting in translation and patient care • Manage your own patient panel as part of a team-based approach to patient care • Develop and cultivate long-term patient relationships • Provide comprehensive healthcare services in a patient-centered medical home environment • Serve a largely migrant and Latino community with compassionate care • Must have completed Master's or Doctoral Degree (Nurse Practitioner) or Master's Degree/Bachelor's Degree in Health Science with a two-year Physician Assistant training program • Participate in a team-based approach to patient care in a community health setting
