Facility Location: Port Angeles, WA State (not a remote position)

Department: Medical Services

Reports to: Chief Medical Officer

Pay Range: $135,000 to $160,000 per year (DOE/DOQ)

Hours per Week: 40 hours

Workdays: Monday to Friday

Job Overview: The Primary Care Provider fulfills a key role in our busy Community Mental Health Center, providing comprehensive healthcare services. This full-time opportunity works within a collaborative team of professionals, offering a wide range of services, from health promotion and disease prevention to direct care and counseling. This position works closely with the Medical Director to establish a fully integrated model of care for our patients.

Essential Duties and Responsibilities:

  • Manages a caseload of patients with acute and chronic disease states with responsibility of overall patient primary care.
  • Conducts physical exams as required.
  • Develops treatment plans for acute and chronic diseases.
  • Educates and guides patients on disease prevention and healthy lifestyle habits.
  • Understands the changes (and challenges) in health promotion throughout the aging process.
  • Performs and interprets diagnostic tests and screening evaluations.
  • Emphasizes preventative care and disease management.
  • Prescribes, administers, and reviews medications of assigned clients. Regularly monitors side effects of prescribed medications. Provides consumers with information necessary to make informed consents.
  • Provide regularly scheduled (usually weekly) caseload consultation to psychiatric peers. Case review consultations focus primarily on those clients seen in both primary care and psychiatry.
  • Develops and delivers in-service training regarding recognition and treatment of physical health conditions in psychiatric patients.
  • Conducts and completes brief health assessments for clients at the Clallam County Respite Center (CCRC).
  • Maintains familiarity with and handles client information in accordance with Federal Regulations (42 CFR, Part 2), the Revised Code of Washington (RCW 71.05.390 and RCW 71.24), and other applicable laws pertaining to confidentiality of client and staff information.
  • Maintains physical security of confidential materials and assigned Agency property.

Qualifications:

  • Degree from an accredited school in one of the following disciplines: Nurse Practitioner (NP), Medical Doctor (MD), or Doctor of Osteopathic Medicine (DO)
  • Current, unrestricted licensure in Washington State as a NP, MD, or DO
  • Preferred: At least four (4) years of experience in treating acute and chronic diseases
